New Patient 101
Patient Consultation
Whether you’re receiving Botox, laser treatments, or microneedling, we require all new patients to undergo a consultation to help us better understand your needs and concerns. This will ensure we provide you with the best possible treatment so you can achieve the results you’re looking for. It’s important to know that all patients who are under the age of 18 must have a parent or guardian with them during their consultation.
Depending on the treatment, we will at times perform the procedure on the same day as the consultation. However, a more in-depth treatment may require a second appointment to continue building out the most effective treatment plan.
What To Bring To Your Consultation
Before attending your consultation, be sure to gather all necessary information so our providers can gain an in-depth understanding of your medical history. This will assist them in building out your treatment plan and make them aware of any underlying conditions or medications that could interfere with the desired plan.
Here are the documents that must be brought to your consultation.
- A list of medications you are currently taking
- Completed medical and dental insurance forms (if applicable)
